Our mission is to improve healthcare for patients experiencing Chronic illnesses and prevent hospital readmissions, between doctor visits. SyncHealth Management has an unorthodox approach that offers care coordination between the physician’s office and the patients home. This approach has improved the care for patients and provided additional reimbursements to Physicians, for services SyncHealth provides.

Our vision is to help Patients become more engaged in their healthcare and to educate Physicians on how to maximize their reimbursement, by reducing hospital readmissions and increasing program participation, for patients in need of Chronic Care Management services.

SyncHealth Management is a team of successful healthcare/business professionals that started working together exclusively in 2021. We started a Chronic Care Management (CCM) pilot program working remotely with patient’s and created various strategies with positive outcomes.

Chronic Care Management During COVID-19 radically changed the medical landscape in the U.S., including how providers and patients make health care decisions. Remote patient monitoring have become more valuable and a more viable alternate to treatment of chronic care patients.

SyncHealth Management created services and support to offer patients the care coordination between office visits. We also created a solution to increase patient engagement, at the same time, maximize reimbursements, without any additional work added to the Provider. In fact, our CCM pilot program, reduced the stressors on Physicians, allowing for a more productive delivery of healthcare services to their patients.
